Choose one flavor variation and prepare its add-in: hull the strawberries, dice the mango, peel the banana, dissolve the coffee, or crush the cookies or pistachios as applicable.

Add 1 cup milk and 1 1/2 cups vanilla ice cream to a blender. Add the chosen flavor ingredients: chocolate syrup for chocolate, strawberries for strawberry, vanilla extract for vanilla, cookies for Oreo, mango for mango, banana for banana, dissolved coffee for coffee, peanut butter for peanut butter, Biscoff spread and cookies for Biscoff, pineapple and coconut cream for piña colada, blueberries for blueberry, or pistachios for pistachio.

Blend on medium-high speed for 30–60 seconds, stopping once to scrape down the sides if needed. Continue until smooth, thick, and creamy; cookie or nut pieces may remain slightly textured if desired.

Taste the milkshake and, if necessary, blend in a little more milk to thin it or a small amount of ice cream to thicken it. Add sweetener only if the fruit is tart or not fully ripe.

Pour immediately into a chilled glass. Top with optional whipped cream and a matching garnish, such as a cookie, strawberry, fruit piece, coffee powder, coconut, or pistachios.
